Check with your airline if they will crate and bring your cat on your flight. And you need to have the animal checked and certified by a vet. Check the rules out first.

Planner is correct. Check with the airline, they should have the requirements. As I remember from over 15 years ago bringing in our 2 cats.
Had list from airline and must have vet sign off no more then 2 weeks before entry. Make several copies. Pre-filled in forms provided by airline. We also had sedatives for the cats and a small squeeze bulb for giving them water. Full cat harness and leash. Boston to Miami to POP so we found places to let them out, drink and another 1/2 pill.
2 approved soft cat carriers that would go under seat and reserved with airline. Most do not allow more then 2 animals in the cabin at a time. Check that IF needed, the flight has an approved live animal area in cargo.
Customs should have a copy of the Vet papers from the airline and you will be required to check in with the customs/vet office with your cat. Normal for them to take some time.... waiting for a "tip".
Hope this helps but please double check since this was over 15 years ago.

IF you can not have your cat in the cabin with you and it must be in the animal area in the hold, you will want a hard carrier.
Since you are coming from Ohio, you will need to check with the airline for each leg coming down and going back.

There are two forms that can be filled out but they have to be done by a USDA certified vet. One is a USDA 7001 form. The other is a Dominican form. Whats the difference? The Dominican form says the cat/dog has had rabies vaccination done at least 30 days prior to entering. The USDA form does not. I have entered with 3 cats using the USDA form and the vet at the airport was only too glad to see my money. I took all of them in soft carriers in the cabin with no problems. Except the carrier counts as a carry on and you pay about $100 for the cat.
