My husband has accepted a job in the Bahamas and the company will be submitting and paying for his owrk permit. Yes it is very expensive $10,000. My question is on the application he needs a medical certificate. I cannot seem to find a form for the doctor to fill out. is there a standard form or is it just a clean bill of health on a doctor's letterhead.

Took us a while to figure it out too. No form. He just needs on letter head from dr. My sons read - My patient____ is in good health and free of communicable diseases, He is able to work in the Bahamas. Please call if you have questions. - Your/his dr. may be familiar with providing this to others.
