I've always had the opinion that a SIN (Social Insurance Number) is a pretty permanent thing. It isn't something you re-apply for (other than to replace lost/broken card) as the number assigned to you stays with you.

You don't say what citizenship you have now. I would think that if you were able to get a SIN back then, you must be some sort of Canadian. IF you are in the US, you can go to Canada with your US passport. Your SIN card has been idle that long and is in 'HIBERNATION' mode. You have to go to an office in Canada to reactivate it. I just did the same for mine after losing the card 8 years ago. I'm in the US.
