Under which Jurisdiction ? In UK is relatively easy to register a new name, US is a bit harder. Not sure in Colombia, but money talks in Colombia.

This is what you need more out less. This was the process in the states in a Colombian Embassy which mirrors Colombia.
The document is called "Escritura Publica", then she will be issued a new birth certificate with her married last name, and afterwards she will have her cedula corrected too. Bear in mind that after her cedula is corrected, she will have to wait at least a period of 2-3 months until her last name has been changed by the Regiatraduria Nacional in Colombia in their main system, so that we can process her new passport afterwards.
The documents that she will need for this procedure are the following:
Authenticated copy of her colombian Registro de Nacimiento (birth certificate from Colombia) Cedula Money order for 384 $ dollars Three passport-size pictures, in colour and with white background Money Order for 41.30 $, fee for her corrected cedula
It is very important that she realizes that her name or last name can only be changed once in her lifetime in Colombia.

Thank you mr bigger jail men for taking the time and all your information,,,it's actually not for a lady. It's for a friend that's been a son for me many years.we are in Cali . It doesn't sound to complicated he has a 6 year old son too. So his name would be changed too. Does a notary handle theese matters do you think it would be cheaper here in Colombia.

Hmmmm, if the child's name is going to be changed, then its not going to be as easy as changing an adult's name. A lot more hassle, as both parents of the boy have to agree, and possibly many other complications too. Better seek legal advice on that.

It depends the birth certificate for a child. If no father use listed it it's way easier. If the fathers name is present, it more expensive and difficult.

I would suggest notary also as a start, and the process of re-issuing birth certificates is fairly common- since you mention the father and son both wanting names changed, I assume key issue would be what the mother says. Keep in mind a lot is also a function of money, really almost anything can be done for a price in Colombia especially dealing with lawyers, notaries and judges.

We have done this in Medellin for our son; so easier than your situation. The process took a few weeks, and from memory we required:
Both parents present to sign documents and present our documentation for authentication, and the babies;
The initial application, and interview followed by submission by the Notary.
The document took a few weeks to be formalized, and we were issued with a MMAS Naturaleza de ACTO: Cambio deNombre en Registro Civil de Nacimiento authorizing a change in the Folio de Registro Civil de Nacimiento.
Once formalized, we were issued with a new birth certificate.
This was at Notaria 20 in Medellin.

I dove further more deeply with cleaning a minor name in Colombia. According to a first hand source stated that it is illegal to change a minorS name. ..see attached his reply to me today.
Hello Dave, my apologies for the late response.
It is forbidden by law to change a minor's last name through the procedure I mentioned for your wife. You will have to begin the adoption process through a lawyer in Colombia. It is only possible to change his last name when he is 18 years old.

In Colombia, the notaries sure do a lot of things. I am in the process myself. Registro de Nacimiento is key.

In my case, I wish to give residency to a minor who is related to me. This is backed up by ADN aka paternity test here in Colombia. No attorney needed I was told by the register's office. However, you must go to where the child was registered on the national registry or so as I was informed. I thought this would be easy as I was under the impression that the National Registry is country wide. Wrong! Between the registry guy buying lottery tickets at the register's office while I tried to be polite and wait for him to finish. I finally just asked him just tell me where to go and I will take care of it. I can now see how dysfunctional this system is.
Please watch out for the attorneys who will tell you that this is a lot of money to accomplish this task as they prey on you for not knowing where to go to ask for the surname change. See decreto 999 of 1998.

That registry is crazy, I ran into that too with a blood type error on birth certificate for me step son was born in Barranquilla.. Had to go back there..
