I have duel British/American citizenship and retired a couple of years ago. We lived in California, my husband is American. We moved to France and I was told that as I receive a British pension, albeit a reduced one, as I was in the USA for 24 years and didn't pay any contributions. I was still entitled to an E121, which also covers my Husband and Grandson. As your wife has paid her contributions it would seem that she is entitled to an E121 when she reaches 60. I would check with whoever it is these days that deals with Health in the UK. If you come before she retires I think you can use the E106 which is valid for two years.
