We had Codice Fiscale # when we last lived in Italy (US citizens supporting the US Military) several years ago. We can't find them in our paperwork now - how hard is it to find them online or will the Italian Consulate in LA, CA be able to find them - this is where we'll file for our Visa. We know we need them for lease documents.

We didn't have them when we arrived, but our landlord arranged for one for my husband through a local CPA. I got mine when they issued me a permesso di soggiorno, it's at the bottom of the card. I imagine an Italian accountant can do the same for you, if you don't find your existing ones.

I would follow the directions here http://www.conslosangeles.esteri.it/consolato_losangeles/en/i_servizi/altri_servizi/codice-fiscale.html with the exception that I would include a cover letter stating that you previously had been issued a codice fiscale however have lost the cards.
Although I have an actual card, issued in early 2009, my wife received only an email stating her number about one year later. It seems that codice fiscale cards are no longer issued; for persons residing in Italy the codice fiscale and the tessera sanitaria (health system card) are combined into a single card.
Is it possible that, like my wife, you never had an actual card? If so, maybe you are looking in the wrong places. :)

A codice fiscale card is still issued to those who do not participate in the national health care system, like me. I have private health care insurance, so I received a codice fiscale card. Foreigners who do participate in the national health care system are issued a combined card.

But, you are already in Italy, correct?
We are still in the US and, as such, I don't think cards are issued in such cases.

I believe that once you arrive in Italy, you can go to the local Agenzia dell'Entrata and ask for the plastic card, if you so desire. After I arrived, I went to their office to get my tax number, then went back later and asked for a card once I knew they would issue one if requested. One isn't required to do so, but it lasts a lot longer, since one frequently has to reference it, than a piece of paper with the number printed on it.

I meant to add that it is also, as proseh58 mentioned, printed on the bottom of one's permesso di soggiorno, so, again, the green plastic card really isn't necessary, but I prefer to have it.

DoppioCittadino, Yep, we had C/Fiscale cards - they were green. We're still going through our paperwork and hope to find them. We've got our Vaporetto ID Cards, punch cards for 10th gelato free at a place near the Trevi Fountain, some Naples bus passes, etc., even some lotto tickets from some festival that are too faded to read, but no C/F card yet. Setting that aside, your instructions are helpful. Thanks for the info.

We found one of our C/Fiscale cards! It's even the plastic one that's got a green stripe across the top. Now to think good thoughts for finding the other one.....
