Does anyone know which countries do NOT require a medical clearance for residency?

You haven't included enough information for anyone to be able to answer your question.
First, what is your nationality? Second, what list of countries do you have that you can qualify for residency for without regard to medical requirements?
There are roughly 200 countries in the world, you need to narrow down the list a bit yourself first. No one is going to know which of the 200 do not require a medical.
It's easy to find out if a specific country requires one or not, it's not easy to find a list of countries that do not.

That can't be your only criteria. Even if Canada didn't require a medical exam, you would still have to qualify in other ways.

Cyprus does not, Slovakia and Bulgaria do. I know this from personal experience.

Thanks for replying and good point! I am an American and I am looking for 3 countries in Asia, Europe, Mexico, Central/South America. So the top 3 for each region - IF ANYONE can assist me!

You are asking people to work in the dark.
Ignoring medical checks for the moment, what countries can you qualify for residency in?
For example, Cyprus has been mentioned. Do you meet their residency requirements? If so, how, in what category? http://cyprus.angloinfo.com/information/moving/residency/non-eu-citizens/
A medical is not the ONLY requirement you have to consider. To put it simply, if you are an unemployed MickeyD's burger flipper with no money, you are unlikely to find anywhere you can get legal residency regardless of whether you need a medical or not.
If you are a retiree with an annual income of $100,000, then that is a different story but you seem to want to keep the 'story' a secret. Without that background story, people have no way of knowing whether they are wasting their time or not in trying to help you with the seemingly 'simple' question you have asked.
