Transfering Polish land from mother to me

My mother lives in Canada, and I live in USA. Both of us are Canadian and Polish citizens. She wants to transfer ownership of land she has in Poland to me, without having to travel to Poland. Can this be done in Canada? If so, what forms are needed and can anyone recommend a good notary or lawyer?

Has anyone here done anything like that?
