Unfortunately Spain and the rest of the EU treat doctors from the US like we treat foreign doctors. You would have to take tests for both licensing (like USMLE) and certifcation (like our ECFMG) and then you would have to repeat specialty training. If you are not looking only at Spain there are developed places around the world like the UAE, NZ, Australia, & Canada that are hiring. Then there is a Spanish fluency requirement. You may have an advantage if you have a degree from a Latin American medical school. Apparently there is no shortage of doctors because compensation is comparatively low. Best to refer to the Spanish government website for more details. An internet search of this topic reveals very little and what is there is old. Teaching may be another option but I have no info.
