Assuming you mean your visa is only good for 30, 90 or 180 days and you must then become a resident or leave the country for 3 days.
Most people go to Costa Rica and vice versa. There is only the one land border crossing at the southern end of the PanAmerican highway. Another is due to open soon but we have been waiting for it for 5 years.
If you come back to Nicaragua you again qualify for the tourist pass for 90 days for $10 I believe. The real cost is spending 3 days in Costa Rica.
While not legal, many people simply go to the border and get someone to stamp your passport accordlingly. I think it is around $40-50 for the bribe (ah, I mean courtesy tip). If not sure where to go just ask any taxi driver.
The border crossing is 1 hr 15 min from Granada or two hours from Managua.
A pleasant trip is to just take the Tico or Nica bus to Costa Rica. Makes the border crossing much more pleasant and the buses are pretty good.
