Hello! I'm hoping to move to Nicaragua, and get out of the California rat race. I see among the papers needed is a "Clean Bill of Health." As someone with a history of breast cancer, I'm concerned that this will be the "fly in the ointment." Can anyone who has seen/filled out/completed this form advise me of what it entails? Thank you!

No worries. All you have to do is get a letter, on doctor's letterhead, stating that you are in general good health and have no communicable diseases. They don't want anyone coming down and sharing. There is no form and no health history is required. The letter can be one sentence. Get the letter notarized and then apostilized (I can't spell that).
We went to a walk-in clinic, had a short physical with blood and urine tests, and got the letter with no problem. It took one hour mostly waiting, and going back the next day for results. I even wrote the sentence for them:
I have examinied so and so on this date and find her/him to be in good health with no communicable diseases.
It is that simple.

I imagine in the US before you leave! They need police record too, and proof of enough $..... I'm going down to Granada in September to check it out...I really think I'm going to love it!

Before you leave the US you will need a notarized letter from your dr. It can say you are in good health and no STDS. You will need a certified copy of your birth certificate. You will need a financial statement explaining where your money will be coming from. A notarized LOCAL police check. Copies of your passport to include the page where you we registered in country. You would probably be better off to make about ten copies of each of the above document before coming here. All the notarized documents will need to be authenticated by the Sec Of State in the state the documents were issued. Going to the Nicaragua Consulate is no longer a requirement. If you are bringing pets, go to the website for bringing pets into Nicaragua and it will give you all the requirements. It has been made much easier in the last two years. There is a lady in Granada. With all due respect, I met with Nadene in Granada in July, so she is still living in Granada, to the best of my knowledge. However, she is no longer facilitating residency applications as she was offered other work. She is finishing her current open cases and not taking on new ones.
Patricia Sanchez in Managua is also a facilitator and can be found on Facebook.
