Jordan: Need import permit application form to bring dog:

I am moving from USA to Jordan in June. I am bringing my dog and the costums told me I need an import permit aplication form. They told me to contact the Ministry of Agriculture, but they have never answered my emails. Does anyone have or know where I can get this form??? Thanks
