if you become a CR citizen do you need to keep Medicare ?

Medicare does not cover anyone outside the US and it's possessions no matter what your spouse is.

As a resident or citizen you need to take part in Costa Rica's government medical system. The system is pretty good, similar to the Canadian system. I believe that there are different levels of coverage, meaning that you can spend more and get more. Two good hospitals available in San Jose, CIMA and Biblica. Biblica is not honest and you have to watch them regarding charges, but the staff are friendlier. Both hospitals have english-speaking staff.

I resided in CR for 3 1/2yrs. Had an appt at CIMA, Internal med Dr and he charged USD150.00 per visit. Locals paid USD75.00. Needless to say I didn't see him any more.
