Change of Residence forms????

We've just moved to the Drome Provencale and are expecting our container of household goods to arrive next week. We've been told that we won't have to pay duty if we meet certain criteria, but there's something called a Change of Residence form that we're supposed to fill out. We don't know what this is or where to get one. The gentleman at the moving company said we should get it from the French Embassy in the US, which doesn't seem to make a lot of sense to us. But we checked their website and didn't find any such document. Does anyone know what this is? Has anyone gone through the experience of bringing in a container and NOT paying duty on it? We'd really appreciate any info. Thanks in advance.
