I’m curious about ophthalmic medical assistant jobs in Costa Rica. Particularly, if I can’t find one immediately, are there cafes or restaurants that pay well enough to afford to live while I look?

This is not 100% true in medical fields and several others you may be hired and extended a work visa. If you ask where you might be hired or Google this field or work visas you will see lists.

The company or hospital will have to extend the visa if the job isn’t able to be filled by Ticos.

You can not work at any job in CR unless you are a resident. This takes years to obtain. The only exception is if you have a job that is solely digital, not physical, and solely computer/web based. They are VERY strict about this.

I am living here full time in Playa Hermosa, Puntarenas

It is my understanding that you cannot hold any job here as an Expat.

You can own a business and then hire locals to work your business.
But jobs like in a cafe or restaurant are not possible for expats.

Jobs go to the locals and for good reason to support the economy.. Something not happening well in the USA.

Digital Nomad is the common expat here.

You can't legally work until you have received Permanent residency which will take approx 4-5 years, work online, have a work visa, are married to a citizen spouse or have a child born there.

Wages ages are very low in CR.

Anyone in the medical field you would be required to be bilingual and be prepared to resit your exams...in Spanish.
