medical certificate for visa

How do we get a medical certificate for a visa? If I go to a clinic and say that I want such a certificate, will they know what I'm talking about? I can imagine each medical facility completing the task differently. Do we use a form provided by the Ecuadorian government? If not, does the certificate need to be translated?
