Stamp O health declaration

I am in the process of putting together all the documentation I need for an application for Permission to Remain and Stamp O, and I have a question: can anyone tell me what form the health declaration is supposed to take? Is there a form somewhere that I need to complete, or do I need to see a GP, or is it something else? Thank you to all for any help you can give me.
