Signing up for Medicare

Soon I will need to sign up for Medicare in the US. I now live in Italy as a legal resident. I'll also want to get a supplemental insurance plan in the US to compliment Medicare. I understand that Medicare can't be used outside the US, but I am entitled to it and feel I should have it as back-up. Has anyone done this online while living in Italy. Did you use a US address (family?). Any issues I should know about.
