Health Insurance Question

If I am living in a Central America country, using their health care system and return to the US for 3 or 4 months out of a year. What is the best method of securing health care insurance for the time I'm in the U.S.? My wife and I are US citizens but will not be of age to enter into the Medicare system for 6 or 7 years.
