Health Insurance For Expats

Ok here is what I hope will not be to complicated a question.

One of the concerns my wife and I have (we are Pre-Medicare) is what happens if we have something pretty major and want to go to a hospital in the US? We need to maintain at least some type of catastrophic plan.

Is there a plan that anyone knows about that would cover both Panama & the US. We don't mind something like a $10,000 deductible but need secure coverage for something major.
