Pets and kidney disease

We're considering moving to Panama or CR. Our older cat has kidney disease, common among cats, and needs a special diet and meds.
Could someone please comment on their experience with vets, special diets, and meds.
I would presume other folks have either been there done that, or doing it now.
Shipping special food will take a special permit from the agricultural department and probably take forever to get. What do you guys do?
Are you satisfied with the vet services provided? How accessible are the vets and special diets?
