90 day visa extension?

My Colombiana friend is a resident of Panama. Her 19 year old
nephew has visited her twice in the past 5 years, each time departing
on time. Since the current rules allow him 90 days per visit, how easy
and what is the cost of receiving an extension before his 90 days expire? thanks in advance for your input on this
