Physical Examination Record for Foreigners and Chengdu and toddlers

HI, what a mouthful!

I have been offered a nice position in Chengdu. I am bringing my daughter, age 4.

I am reading the Physical Examination form and wondering, do we really need all these tests or can my physician sign off on all of this? Do I need receipts showing that we had these procedures? What does the consulate expect from my physician in terms of answers? I.E. Lungs: Clear.?

I don't have insurance and can't afford an ECG. ( the average cost is $550)

Any suggestions? How did you handle this one?
