Water in Cartagena. Does the stomach ever adjust?

I was in Cartagena a few weeks ago. I wound up getting sick to my stomach (Diarrhea) . I only ate at touristy places, but I did have some margaritas and they did have ice in them. I've had this happen to me several times over the decades in both Barranquilla & Cartagena.

My question is for the expats. Has your stomach adjusted to the water in these coastal cities? At some point, I'd consider residing there, but there is no way I'd want to have a permanent case of diarrhea. If I were living there, I think I'd always be somewhat cautious and drink bottled water, but it seems to be somewhat unavoidable to come in contact with tap water in one form or another. If that is the case, either my stomach would have to adjust, or I couldn't enjoy living there.
