Insurance Question San Jose

Hello all. I have a simple question which likely has a complex answer that I hope you can help me with. I am probably a couple of years out from retirement, but may be able to do the digital nomad thing sooner. Ok, I get that if you are a permanent resident you pay into Caja which is great, I guess. But in my researching all I ever seem to read is that in general it is pretty good and in San Jose (area) top notch, but hit or miss on wait times and availability of services. To my question, if you had a heart attack in San Jose is service fast and safe in Caja?
I will likely ask a similar question on Private insurance, but there does seem to be more information on the private side on the hospital web sites (like CIMA or Biblica).
