ATM's and the Strength of the Dollar

Excuse my ignorance but I have several questions regarding daily living in Ecuador. 1.) Because Ecuador is a mostly cash society I have read it may be difficult to obtain small US currency such as $1's and $5's, is this true? 2.) If yes, how does a person gain access to needed small bills? 3.) Do ATM's distribute them? 4.) Are ATM's plentiful and well stocked? 5.) Being that Ecuador uses the US dollar and the dollar having strengthened against other currencies has this affected your purchasing power in a positive way in Ecuador? 6.) Can a person live without a car in Ecuador? 7.) What is the accepted practice regarding tipping in Ecuador, is it a tipping culture for services? 8.) Are shoes for people with wide feet readily available, I am a man's size 11, 6E?
