How small can you go city/town before key amenities drop off

Soy Canadiense, un jubilado. I'm 71 in excellent health. We're planning a move to Ecuador. Right now we live in the tiny City of Summerside, Prince Edward Island, Canada, population 16,000.

My question has to do with demographics and amenities key to senior citizens--reasonably comprehensive health care, good public transit, walkability/bike-ability, a university (I'm an armchair academic). I don't give a hoot about restaurants, nightclubs, but I do like sports., especially tennis. Also, the arts, music, fine arts, history, dance, theatre, yes, yes... Interested in community initiatives and local government participation, perhaps on a committee or an alcade's task force.

Are there cities besides the usual expat havens, perhaps a little smaller or a lot smaller, wherein one can still partake of the metropolitan offerings? Having expat support is nice but doesn't really excite me much.

Loja, arts and culture capital, so named, looks attractive.

Any ideas? Suggestions?
