Health Insurance Costs

As I prepare to take the leap of becoming an expat in the next year or two, health insurance costs are the biggest mystery. It would be awesome if people were willing to share their expat. healthcare costs in a way that made it somewhat easy to compare between policies since not all coverage is equal.

Real Life Example 1: US based healthcare plan through my employer for just me
(1) cost per month: $757 USD / 91% ($689) paid by employer / 9% ($68) paid by me
(2) # of people covered: 1
(3) what is covered: preventive care 100% (annual checkup, routine immunizations/shots, women's gyno exam, mammograms, colorectal exams)
(4) what isn't covered: vision & dental are separate plans;
(5) deductible: $1500 per calendar year, then I pay 20% in-network or 40% out-of-network until out of pocket max is reached.
(6) prescription drugs: pay 100% until deductible is met, then 20% after until out of pocket max is reached.
(7) out of pocket maximum: $3000 per calendar year, after which the plan pays 100%
(8) Who is the coverage through: Providence Health Plan

Real Life Example 2: US based healthcare plan through my employer for employee, spouse/partner, and children.
(1) cost per month: $1826.95 USD / 80% ($1462) paid by employer / 20% ($354) paid by employee
(2) # of people covered: family (employee + spouse/domestic partner + children
(3) what is covered: preventive care 100% (annual checkup, routine immunizations/shots, women's gyno exam, mammograms, colorectal exams)
(4) what isn't covered: vision & dental are separate plans;
(5) deductible: $1500 per calendar year, then I pay 20% in-network or 40% out-of-network until out of pocket max is reached.
