One question I’ve been scared to ask… I am terrified of cockroaches. Seriously. Bat shit crazy. I freeze, hyperventilate… How bad are they in Costa Rica?

A (relatively) short answer.
They are everywhere in the world. All climates. I have never seen one in my time in the South Pacific in a house.
They are in Antarctica for G-d? sake .
So, I am sure they are there.
It is the tropics, an easy place for many things to live.
Where you live and how you live, and how people near you in an apartment or neighborhood live will all have an effect on what cockroaches think about the area you exist in.

In 3 years have never seen one in our condo in Escazu, nor anywhere we have stayed (including the beaches). In fact we have rarely seen any bugs, spiders, etc. (lots of ants though), but we always stay in nice places that I am guessing use a pest control service? On condo development does. Kind of weird now when I think about it, but we saw far more "bugs" in San Diego where we are from then here.

I'm going to be specific. I live just outside of Montezuma in a two bedroom casa. We are surrounded by jungle and yes, we have bugs, all kinds of bugs, plus scorpions, tarantulas and lots of arachnids. I'm not a fan of cockroaches and find them often. Small, medium and large. Do we spray, of course. Now, having said this the natural beauty that supports all these local inhabitants is incredible. You have to understand the balance.
