Need to renew 5 yr old daughter's US passport

I think I have to make an appointment with the embassy in Quito to renew my 5 yr old daughter's US passport. But it really wasn't clear what forms and if she and my husband needed to come as well. We hate traveling all the way to Quito, have jobs and other small children and she doesn't travel well, not to mention she would have to miss school. Has anyone done this recently? What is required? I sent off a note to them, but maybe one of you could fill me in. THanks. Michelle
